How the practice works
Exceptions identify the standard being varied, the reason, the affected operation and the condition for review. Some are enduring and justified. Others are inherited workarounds whose original explanation has become difficult to locate.

A working example
A local reporting process needs a different timetable. Its owner documents the requirement and the interface with the common service. Another exception exists only because an old system was never retired; it receives a funded resolution plan.
