The Usual extension
Usual adds a pre-improvement authorization layer to the familiar DMAIC improvement cycle. Teams first establish whether the process is approved to be described as defective. The Improvement Eligibility Panel then determines whether reducing the defect would affect an existing performance narrative.

Inside the process
A team proposes to remove a duplicate entry. The panel requests a baseline of duplicate entries, confirmation that the duplication is unintentional and approval from both owners of the same field. The improvement remains promising pending agreement on which entry is the duplicate.
