The model
Classify meeting output as a decision, a resolved question, an assigned action or shared context. All four can be legitimate. The model asks whether the stated purpose matches what happened and whether the output can be found by someone who was not present.

In practice
A weekly steering meeting produces excellent context but no decisions. Renaming it as a briefing clarifies its purpose; decisions move to a smaller session with the right authority. Attendance drops without anyone having to declare a cultural transformation.
