The sequence
A function requires staff to run every draft through an approved model. No task boundary is set. Colleagues submit routine acknowledgements, then copy outputs back unchanged so the adoption dashboard turns green. One team stops reading customer exceptions because the generated summaries are shorter than the underlying cases.
The exposure
The missing exception is replicated through hundreds of customer responses. Managers point to their perfect usage rate; Internal Audit finds no evidence that the underlying cases were read. The business must reopen affected commitments, calculate remedies and explain why its own adoption measure rewarded the process that missed them. A green chart becomes an unusually durable piece of evidence against the company.
The control point
Mandatory use applies only to a defined registered workflow. The reviewer must compare output with the underlying case and may choose the manual route when the exception requires it. A prompt count cannot establish that a decision was understood.
Training question
Which failure was caused by insufficient AI use and which by excessive reliance? It is possible for a team to comply with an adoption metric while breaching the actual review standard.
