The model
Every exception records its reason, owner, affected users and return condition. Review occurs when the condition changes, not only when an annual list is requested. Some exceptions should remain; the model makes their continued existence a decision.

In practice
A regional process cannot use the common tool because of a legitimate local requirement. The exception is retained and supported. Another survives because its original owner has left; that one returns for review rather than automatically inheriting permanence.
